Position Overview

Integrative Emergency Services is in search of a Patient Care Facilitator with a background in Medical Assistance or Licensed Vocational Nursing. This role encompasses both administrative and clinical responsibilities under the supervision of a licensed healthcare professional. The Patient Care Facilitator plays a crucial role in enhancing communication between patients and healthcare providers to ensure exceptional patient care.

Key Responsibilities

Responsibilities include, but are not limited to:
  • Regularly assess assigned patients to conduct timely and appropriate care outreach.
  • Coordinate routine telehealth appointments with healthcare providers.
  • Facilitate communication with healthcare providers, including external specialists involved in patient care.
  • Assist in obtaining patient medical records.
  • Develop and maintain care plans based on provider-approved strategies for patient-centered care.
  • Ensure care plans are communicated to patients and families in accordance with regulatory guidelines.
  • Provide education to patients and their families.
  • Ensure adherence to regulatory standards for Chronic Care Management.
  • Assist in onboarding new patients into the care management program.
  • Collaborate with interdisciplinary teams, including social workers and home health providers, to address patient needs.
  • Promptly report any errors or issues to the supervisor.
  • Safeguard the confidentiality of medical records and patient health information in compliance with privacy standards.
  • Communicate urgent concerns regarding patients to healthcare providers.
  • Address patient inquiries, requests, and complaints, escalating complex issues as necessary.
  • Provide occasional guidance, technical support, and training to staff.
  • Perform additional duties as assigned by the Patient Care Facilitator.

Qualifications

Knowledge, Skills, and Abilities:
To excel in this role, candidates must demonstrate proficiency in the following areas:

  • Effective teamwork capabilities.

  • Proficiency in MS Office applications and internet navigation.
  • Proficient in reading, writing, and speaking English.
  • Knowledge of and adherence to privacy standards and confidentiality protocols.
  • Strong listening skills and the ability to empathize with a diverse patient population and their families.
  • Ability to effectively present information and respond to inquiries from various stakeholders.

Education / Experience:Minimum education, technical training, and/or experience preferred for this position:
  • At least 2 years of experience in health education, patient navigation, or health clinic assistance.
  • High School Diploma or equivalent.
  • Valid driver's license.
  • Reliable transportation.

Preferred Qualifications:
  • Certification in Medical Assisting.
  • Active nursing license in good standing.
  • Associate's Degree.
  • Bilingual proficiency (e.g., Vietnamese, Chinese, Arabic, and/or Spanish in addition to English).

Physical Requirements:The physical demands necessary to perform the essential functions of this role include:
  • Ability to see at various distances and adjust focus.
  • Regularly required to stand, sit, talk, hear, and use hands for computer and telephone tasks.
  • Frequent walking and the ability to navigate healthcare facilities.
  • Capability to perform standard office tasks, including sitting for extended periods, filing, and lifting office supplies.
  • Light to moderate lifting (up to 25 pounds).
  • Fine motor skills for keyboarding and other tasks.
  • Ability to manage the emotional demands of assisting patients from diverse backgrounds.

Work Environment:The work environment characteristics encountered while performing this role include:
  • Typically low noise levels.
  • Standard office environment without hazardous conditions.
  • Compliance with workplace policies regarding smoking and health regulations.
  • Adherence to any immunization or vaccination requirements as per client policies.

Compensation:
  • Hourly wage range of $18-$25.

Travel Requirements:
  • Local travel may be required for office-related tasks.
  • Work hours are typically Monday through Friday, 8 AM to 5 PM.
