Position Overview:

The Clinical Transition Educator plays a crucial role in facilitating the effective transition of patients to the innovative care delivery model at Option Care Health. This position is dedicated to enhancing the educational experience for patients, caregivers, and healthcare facility staff regarding the services and products offered by Option Care Health.

Key Responsibilities:
  • Conduct comprehensive evaluations and provide educational training to ensure a seamless transition for patients into the Option Care Health delivery framework.
  • Collaborate closely with the sales team to identify opportunities for increasing patient transitions to our care model.
  • Maintain strict confidentiality regarding patient and proprietary information, adhering to all legal guidelines.
  • Document educational activities accurately using Option Care Health's technology and applications.
Qualifications:

Basic Requirements:

  • Active and unrestricted Registered Nurse (RN) license.
  • A minimum of two years of experience in the healthcare sector.
  • Proven ability to build and maintain relationships across various levels within the organization and with external partners.
  • Familiarity with current industry standards and practices relevant to healthcare services.
  • Strong customer service skills, ensuring high-quality service delivery and customer satisfaction.
  • Basic proficiency in Microsoft Office Suite and other relevant software.
  • Ability to travel locally to various healthcare facilities and patient homes.

Preferred Qualifications:

  • Experience in home or alternate site infusion therapy.
  • Background in developing partnerships with service providers.
