Parkland Health and Hospital System (PHHS) is seeking a skilled Patient Navigator to join our team in the Correctional Health department. As a Patient Navigator, you will play a vital role in ensuring that patients receive timely and quality medical care, while also addressing their social and economic needs.

Key Responsibilities
  1. Communicate Effectively: Collaborate with patients, caregivers, and healthcare teams to ensure patient adherence to care plans, while advocating for their needs, rights, and cultural preferences.
  2. Promote Collaborative Practice: Foster a collaborative environment among patients, families, and multidisciplinary healthcare team members, serving as a resource person for clinical and patient care issues.
  3. Conduct Patient Assessments: Evaluate patient needs and refer them to appropriate resources to facilitate access to care, such as transportation, childcare, and financial counseling.
  4. Provide Education and Support: Educate patients and communities about Parkland services, referral processes, authorization processes, plan coverage, and community resources.
  5. Coordinate Care: Refer patients to primary care, specialty providers, or other healthcare services to ensure quality and continuity of care.
  6. Document and Report: Maintain accurate documentation in EPIC and other relevant software, while supporting required reporting for accreditations, grants, and other entities.
  7. Stay Informed and Compliant: Stay up-to-date on applicable rules, regulations, policies, laws, and guidelines that impact the department, and develop effective internal controls to promote adherence.
  8. Continuously Improve: Identify and implement process improvement opportunities, while developing training and monitoring annual goals and objectives.
