Key Responsibilities- Communicate Effectively: Collaborate with patients, caregivers, and healthcare teams to ensure patient adherence to care plans, while advocating for their needs, rights, and cultural preferences.
- Promote Collaborative Practice: Foster a collaborative environment among patients, families, and multidisciplinary healthcare team members, serving as a resource person for clinical and patient care issues.
- Conduct Patient Assessments: Evaluate patient needs and refer them to appropriate resources to facilitate access to care, such as transportation, childcare, and financial counseling.
- Provide Education and Support: Educate patients and communities about Parkland services, referral processes, authorization processes, plan coverage, and community resources.
- Coordinate Care: Refer patients to primary care, specialty providers, or other healthcare services to ensure quality and continuity of care.
- Document and Report: Maintain accurate documentation in EPIC and other relevant software, while supporting required reporting for accreditations, grants, and other entities.
- Stay Informed and Compliant: Stay up-to-date on applicable rules, regulations, policies, laws, and guidelines that impact the department, and develop effective internal controls to promote adherence.
- Continuously Improve: Identify and implement process improvement opportunities, while developing training and monitoring annual goals and objectives.
