POSITION SUMMARY
Integrative Emergency Services is in search of a dedicated Care Coordinator with a background in Medical Assisting or Licensed Vocational Nursing. The Care Coordinator plays a crucial role in executing both administrative and clinical responsibilities under the supervision of a licensed healthcare professional. This position is essential in enhancing communication between patients and healthcare providers to ensure optimal patient care.

This role will primarily focus on supporting patient care initiatives.

We are committed to empowering and assisting patients and families navigating chronic or terminal illnesses, enabling informed decisions throughout their healthcare journey.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Responsibilities include, but are not limited to:
  • Regularly assess assigned patients to conduct timely and appropriate care calls.
  • Coordinate routine tele-visits with healthcare providers.
  • Facilitate communication with providers, including specialists involved in patient care.
  • Assist in obtaining patient records and maintain care plans aligned with provider-approved strategies.
  • Ensure care plans are effectively communicated to patients and families, adhering to regulatory guidelines.
  • Provide education to patients and families regarding their care.
  • Ensure compliance with guidelines for Chronic Care Management.
  • Support the enrollment of new patients into care programs.
  • Collaborate with interdisciplinary teams to address patient needs.
  • Report any discrepancies or issues to the supervisor promptly.
  • Maintain the confidentiality and security of medical records in accordance with privacy standards.
  • Address immediate patient concerns and follow up on inquiries or complaints.
  • Offer guidance and training to staff as needed.
QUALIFICATIONS
To be successful in this role, candidates should possess:
  • Strong analytical, organizational, and communication skills.
  • Attention to detail and the ability to manage multiple priorities.
  • Experience working effectively in a team environment.
  • Customer service orientation and proficiency in MS Office applications.
  • Ability to communicate proficiently in English.
  • Knowledge of HIPAA regulations and commitment to maintaining confidentiality.
  • Compassionate listening skills to support a diverse patient population.
  • Ability to present information clearly and respond to inquiries from various stakeholders.
EDUCATION / EXPERIENCE
Required:
  • Minimum of 2 years in health education, patient navigation, or similar fields.
  • High School Diploma or GED.
  • Valid driver’s license and reliable transportation.
PREFERRED:
  • Certification in Medical Assisting.
  • Active nursing license in good standing.
  • Associate's Degree.
  • Bilingual abilities in languages such as Vietnamese, Chinese, Arabic, or Spanish.
PHYSICAL DEMANDS
The physical requirements for this position include:
  • Ability to see at various distances and adjust focus.
  • Regularly required to stand, sit, talk, hear, and use hands for computer and telephone tasks.
  • Capability to lift and carry office supplies as needed.
  • Ability to manage the stress associated with assisting patients from diverse backgrounds.
WORK ENVIRONMENT
The work environment typically includes:
  • A low noise level.
  • A standard office setting free from hazardous conditions.
  • Compliance with immunization and vaccination requirements as per client policies.
EXPECTED PAY:
Compensation ranges from 18 to 25 USD per hour.
