Housing Case Manager Job Description

Job Summary:

We are seeking a highly motivated and compassionate Housing Case Manager to join our team at The Salvation Army Central Territory. As a key member of our housing program, you will play a vital role in assisting clients living in our housing program to access external resources, maintain housing stability, and successfully transition into permanent housing.

Key Responsibilities:

  • Conduct client interviews and case management to determine eligibility for programs and provide options and referrals.
  • Maintain required statistics, including TSA stats and client files.
  • Participate in office and department meetings, committees, and other meetings as appropriate.
  • Network with community resources to help meet clients' additional needs.
  • Assist clients with direct assistance, such as food shelf and voucher programs.
  • Develop and maintain positive relationships with clients and outside service providers.
  • Input all client information into Service Point.
  • Communicate unit requirements and policy changes to staff.
  • Ensure proper documentation of all client interactions.
  • Encourage excellence and increase staff knowledge through proper training and supervision.

Requirements:

  • Bachelor's degree in Social Work or a related field.
  • Experience working as a case worker/case manager, advocate, or resident assistant preferred.
  • Ability to work with a team of co-workers and respect confidentiality.
  • Must be willing to support the mission of The Salvation Army and possess a valid MN driver's license without restrictions.

Working Conditions:

This position works in an independent manner with little direct supervision of the Lead Housing Case Worker. There are normally no hazardous or significantly unpleasant physical working conditions, with the exception of occasional emotional distress or turmoil with and between staff and/or clients.
