Job Title: Supportive Housing Case Manager

Agate Housing and Services is seeking a highly motivated and compassionate Supportive Housing Case Manager to join our team. As a key member of our Housing Programs Department, you will play a vital role in providing ongoing, structured home- and community-based case management services to participants in our Supportive Housing program.

Key Responsibilities:
  • Provide home- and community-based case management services to participants to assist with finding and maintaining housing.
  • Manage multiple sources of funds to assist participants and maintain accurate records.
  • Partner with Housing Locator to assist participants in finding and maintaining housing.
  • Build relationships with landlords and provide landlord-tenant support and mediation.
  • Maintain participant files and complete accurate and timely documentation.
  • Offer regular and quality contact with participants to maintain trust and assist in crisis prevention.
  • Conduct monthly home visits with each assigned participant.
  • Develop individual goal plans with participants and provide supports necessary to achieve participant-identified goals.
Requirements:
  • Valid Driver's License and access to an insured, safe, and reliable vehicle.
  • Pass a DHS NetStudy 2.0 background check.
  • Commitment to Agate Housing and Services' values and mission of ending homelessness and relieving hunger through action and advocacy.
  • Commitment to working towards an anti-racist culture both within Agate Housing and Services and the broader community.
  • Ability to work respectfully with participants, coworkers, and key stakeholders who are diverse with respect to race, ethnicity, gender identity, sexual orientation, socio-economic status, and religion.
  • Ability to establish rapport quickly with people experiencing a crisis through use of trauma-informed care, participant-centered care, and strengths-based approaches.
  • Ability to meet expectations regarding paperwork and documentation through organizational skills, a strong attention to detail, and the ability to work efficiently and accurately.
Preferred Qualifications:
  • One year casework experience.
  • Two years' experience working with participants experiencing homelessness.
  • Licensed Social Worker.
  • Fluency in language(s) used by our participants (Spanish, Amharic, Oromo, Somali, ASL).
