Job Summary

We are seeking a highly motivated and compassionate Housing Case Manager to join our team at The Salvation Army USA Central Territory. As a key member of our Housing Program, you will play a vital role in assisting clients living in our housing program, accessing external resources, maintaining housing stability, and successfully transitioning into permanent housing.

Key Responsibilities
  • Conduct client interviews and case management to determine eligibility for programs, assist in application, provide options, and make referrals.
  • Maintain required statistics, including TSA stats, client files, and other relevant data.
  • Participate in office and department meetings, committees, and other meetings to stay informed and contribute to the team's efforts.
  • Network with community resources to help meet clients' additional needs, such as bus tokens, First Call for Help, and Hennepin County services.
  • Assist clients with direct assistance, including foodshelf vouchers and other essential services.
  • Develop and maintain positive relationships with clients and outside service providers to ensure effective collaboration and support.
  • Input all client information into Service Point and maintain accurate records of client interactions.
  • Communicate unit requirements and policy changes to staff and ensure proper documentation of all client interactions.
  • Encourage excellence and increase staff knowledge through proper training and supervision.
  • Orient new staff to The Salvation Army's and department's policies and procedures.
  • Maintain active interest and involvement in issues related to homelessness, chemical dependency, and mental health.
  • Inform Housing Program Managers of programmatic issues and concerns regarding client health, safety, and well-being.
  • Provide crisis intervention assistance when needed and make home visits to clients when appropriate.
  • Provide required statistics to Lead Housing Case Manager and participate in office and department meetings, committees, and other meetings as appropriate.
  • Data entry, computer updating, and other department functions as assigned.
Requirements

We are looking for a highly motivated and compassionate individual with a Bachelor's degree in Social Work or a related field. Experience working as a case worker/case manager, advocate, or resident assistant is preferred. The ideal candidate will have a good understanding of the delivery of Social Services and community services, the ability to accept supervision and work with a team of co-workers, work well with clients of diverse cultural and socioeconomic backgrounds, respect confidentiality, and be able to work within ethical and legal parameters of social services.

The successful candidate will be able to support the mission of The Salvation Army, be certified in Adult CPR and First Aid, pass background checks and a pre-employment drug test, possess a valid MN driver's license without restrictions, meet MVR driver's qualifications, and complete the required Service Point training course within 30 days of hire.

Working Conditions

The duties and responsibilities of this position cannot be performed from home. There are normally no hazardous or significantly unpleasant physical working conditions, with the exception of occasional emotional distress or turmoil due to varying situations with and between staff and/or clients.
