Job Description

As a Housing Stability Case Manager at NorthPoint Health & Wellness Center, you will play a vital role in creating a healthier community by providing comprehensive case management services to individuals and families at risk of homelessness. Your expertise will be instrumental in developing and implementing housing stability plans, connecting clients with essential resources, and fostering strong relationships with community partners.


Key Responsibilities:
  • Conduct thorough assessments to identify clients' needs and develop personalized housing stability plans.
  • Collaborate with clients, community partners, and internal stakeholders to ensure seamless service delivery and achieve desired outcomes.
  • Provide education and support to clients on topics such as budgeting, financial capability, and healthy relationships.
  • Negotiate lease terms, fees, and deposits with landlords to secure stable housing for clients.
  • Document client progress, maintain accurate records, and ensure timely reporting to stakeholders.
  • Stay up-to-date with local housing regulations, community resources, and best practices in case management.

Requirements:
  • Bachelor's degree in Psychology, Sociology, or a related field (preferred) or an Associate's degree with relevant experience.
  • At least two years of experience in case management, preferably in a non-profit setting working with marginalized communities.
  • Knowledge of HUD, HMIS, and City of Minneapolis housing regulations (preferred).
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work effectively in a fast-paced environment, prioritize tasks, and manage multiple cases simultaneously.
  • Valid driver's license, proof of current insurance, and reliable transportation required.
  • Proficiency in MS Word, MS Excel, HMIS, and computer client tracking databases (experience with Allita a plus).
  • Spanish bilingual skills a plus.
