About the Role

The Housing Stability Case Manager will play a vital role in increasing long-term housing stability in the community and preventing future periods of homelessness. This position will coordinate, facilitate, and support individuals and families to stabilize, maintain, and improve their current housing.

Key Responsibilities
  • Develop and implement housing stability plans and goals that help expedite information, education, and resource flows.
  • Provide case management services to individuals and families at risk of becoming homeless, including skill building, guest advocacy, and awareness of current NorthPoint programs, services, and resources.
  • Collaborate with community partners to support people in reaching their full health potential.
  • Negotiate lease terms, fees, and deposits with potential landlords to secure stable housing for clients.
  • Develop and maintain rapport with individuals and families to increase personal empowerment and self-sufficiency.
Requirements
  • Bachelor's degree in Psychology, Sociology, or a closely related field (preferred).
  • Associates Degree with two years of case management experience in a non-profit setting focusing on marginalized communities, or a High School Diploma or GED with four years' experience in case management in a non-profit setting working with marginalized communities.
  • Two or more years of experience facilitating, coordinating, and/or leading weekly trauma-informed empowerment workshops/groups covering topics such as trauma, healthy relationships, financial capability, and anger management.
  • Knowledge of HUD, HMIS, and City of Minneapolis housing regulations (preferred).
  • Valid driver's license, proof of current insurance, and reliable transportation required.
