Job Summary

We are seeking a highly motivated and compassionate Case Manager to join our team. As a Case Manager, you will work with a diverse group of clients, providing individualized support and advocacy to help them achieve their goals and improve their overall well-being.

Key Responsibilities
  • Provide comprehensive case management services to clients, including outreach, assessment, goal planning, and crisis intervention.
  • Develop and maintain relationships with community resources and service providers to ensure clients have access to necessary support services.
  • Collaborate with other team members to ensure a comprehensive and coordinated approach to client services.
  • Maintain accurate and timely documentation of client interactions and progress.
Requirements
  • Bachelor's degree in human services or a related field.
  • At least three years of experience working with individuals who have experienced homelessness or personal experience with homelessness.
  • Knowledge of domestic abuse, childhood trauma, grief and loss, sexual violence and exploitation, homelessness, social support systems, advocacy, tenant law, and case management.
  • Experience working with mental health and chemical health issues.
  • Computer proficiency and ability to maintain accurate records.
  • Valid driver's license and reliable transportation.
Preferred Qualifications
  • Experience working with individuals who are homeless or have personally experienced homelessness.
  • Bilingual skills.
