Highland Springs is a picturesque 89-acre continuing care retirement community situated in North Dallas, Texas. We are part of a growing network of communities developed and managed by Erickson Senior Living, a national leader in senior living and healthcare with campuses across multiple states.

We are seeking a Clinical Liaison for our Continuing Care Retirement Community (CCRC). In this role, you will oversee the collection and coordination of admissions for referrals from both inpatient acute care and sub-acute care environments, including current residents of the Erickson Senior Living community.

Your collaboration will extend to Physicians, Continuing Care Sales/Admissions Staff, Hospital Case Managers, Social Workers, and Discharge Planners to streamline the admissions and transfer process.


Key Responsibilities:
  • Conduct onsite clinical data collection for referrals to assess suitability for transfer to Continuing Care.
  • Facilitate discharge planning and care coordination for residents transitioning from acute hospital settings to Continuing Care or returning to Independent Living within the Erickson Living community.
  • Engage with physicians regarding expected discharges from healthcare facilities.
  • Identify quality concerns and participate in quality management initiatives alongside the Administrator and/or Medical Director.

Compensation:

Compensation is commensurate with experience, starting at $55,000.

Qualifications:
  • A Bachelor's degree is preferred.
  • 3+ years of clinical experience in an acute hospital setting is preferred.
  • Experience as a liaison or a background in healthcare marketing is advantageous.
  • Familiarity with acute hospitals, rehabilitation hospitals, sub-acute rehabilitation facilities, and the local healthcare market is preferred.
