WORK SCHEDULE

Monday to Friday, 9:00 AM - 5:00 PM

POSITION SUMMARY

We are currently seeking a full-time Case Manager for our Assertive Community Treatment (ACT) program. The Case Manager will engage in home visits and provide educational support to clients. This role also involves developing treatment plans and facilitating connections to appropriate treatment facilities.

KEY RESPONSIBILITIES

  • Complete Entitlement Applications for clients.
  • Accompany clients to Entitlement Offices.
  • Provide guidance on money management and budgeting.
  • Ensure clients maintain active entitlements, including Medicaid and other benefits.
  • Deliver psycho-educational support to families and clients.
  • Facilitate group sessions.
  • Conduct client assessments utilizing relapse prevention strategies.
  • Draft comprehensive treatment plans.
  • Utilize motivational interviewing techniques.
  • Connect clients with treatment facilities.
  • Communicate with managed care organizations.
  • Assist clients in applying for benefits (e.g., Medicaid, SSI/SSD, HEAP, Food Stamps).
  • Provide crisis intervention services.
  • Manage the crisis phone line.
  • Participate in daily team meetings and case discussions.
  • Conduct regular home visits.
  • Perform other duties as assigned.

QUALIFICATIONS

A high school diploma or equivalent is required; a Bachelor's degree is preferred.

At least one year of experience working with the severely and persistently mentally ill (SPMI) population, along with knowledge of community resources such as DSS, SSI, Medicaid Managed Care, and Housing, is essential.

Strong organizational and interpersonal skills are required.

Excellent verbal and written communication skills are necessary.

Ability to manage multiple tasks and prioritize effectively in a dynamic environment.

Proficiency in computer applications, including Microsoft Office, is required.

A valid and clean New York State Driver's License is mandatory.

PHYSICAL DEMANDS

This role involves prolonged periods of sitting and travel for home visits. The ability to lift up to 10 lbs. is required.
