Position Overview
Family Service League is currently seeking a full-time Case Manager for our Assertive Community Treatment (ACT) program. The Case Manager is responsible for conducting home visits and providing educational support to clients. Additionally, the Case Manager develops treatment plans and facilitates connections to treatment facilities.

Key Responsibilities

  • Complete Entitlement Applications.
  • Accompany clients to Entitlement Offices.
  • Provide money management and budgeting education.
  • Ensure active entitlements, including Medicaid and/or other benefits.
  • Deliver psycho-education to families and clients.
  • Facilitate group sessions.
  • Conduct client assessments utilizing relapse prevention plans.
  • Develop and write treatment plans.
  • Employ motivational interviewing techniques.
  • Connect clients to appropriate treatment facilities.
  • Communicate with managed care companies.
  • Assist clients in applying for benefits (e.g., Medicaid, SSI/SSD, HEAP, Food Stamps).
  • Provide crisis intervention services.
  • Cover the crisis phone as needed.
  • Participate in daily team meetings and case conferences.
  • Conduct daily home visits.
  • Perform other duties as assigned.

Qualifications
A high school diploma or equivalent is required; a Bachelor's degree is preferred. Candidates should have at least one year of experience working with the severely and persistently mentally ill (SPMI) population and be familiar with community resources, including DSS, SSI, Medicaid Managed Care, and Housing. Strong organizational and interpersonal skills are essential, along with excellent verbal and written communication abilities. Candidates must be able to multitask and prioritize effectively in a dynamic environment. Proficiency in computer skills, including Microsoft Office, is required. A valid and clean New York State Driver's License is also necessary.

Physical Requirements
This role requires prolonged periods of sitting and travel for home visits. Candidates must be able to lift up to 10 lbs.
