Job Summary

Family Service League Inc. is seeking a highly skilled Case Manager to join our team in the Adult Care Management State Funded program. As a Case Manager, you will be responsible for providing intensive support services to individuals with serious mental illnesses and other complex needs.

Key Responsibilities
  • Develop and implement individualized service plans to meet the unique needs of each client.
  • Conduct regular home visits and community meetings to assess client needs and provide support.
  • Collaborate with mental health, medical, and substance abuse providers to ensure comprehensive care.
  • Maintain accurate and timely documentation of client progress and services provided.
  • Report incidents and concerns to program management staff in a timely manner.
  • Adhere to all agency and program mandates, including those set by FSL, Department of Health, and Suffolk County Division of Mental Hygiene.
Requirements
  • Bachelor's Degree in a health-related field, such as social work, psychology, or counseling.
  • Minimum of 2 years of experience providing direct services to individuals with serious mental illness.
  • Proficiency in Microsoft Office and experience with electronic health records (EHRs) is preferred.
  • Strong assessment, engagement, organizational, and time management skills.
  • Excellent verbal and written communication skills.
  • Valid and clean New York State Driver's License.
