The Patient Flow Logistics Coordinator plays a crucial role within the Logistics Center, overseeing the comprehensive management of patient flow into, throughout, and out of the healthcare facility. This position requires a blend of clinical knowledge, leadership capabilities, and exceptional communication skills, alongside the ability to effectively utilize technology and data analytics for prompt decision-making regarding patient flow. Responsibilities include managing admissions through various entry points, overseeing transfers, bed management, and optimizing overall throughput.

Key Responsibilities:

  • Reports directly to the Logistics Center.
  • Oversees patient flow management into, through, and out of the healthcare facility.
  • Responsible for patient placement for all admissions via multiple entry points (transfers, emergency department, operating room, direct admissions).
  • Manages overall bed availability and capacity.
  • Monitors and manages patient capacity effectively.
  • Facilitates transfers within the Albany Med Health System and to external organizations.
  • Coordinates consults as necessary.
  • Employs clinical expertise and data analytics to make timely assessments and decisions regarding patient flow.
  • Utilizes strong communication and leadership skills to enhance access, capacity, efficiency, and ensure high-quality patient care.
  • Proactively identifies and mitigates potential patient flow barriers.
  • Escalates unresolved barriers through the appropriate chain of command within the Logistics Center.
  • Comfortable with electronic medical records and technology tools used in the Logistics Center.

Qualifications:

  • Associate's Degree - required
  • Bachelor's Degree - preferred
  • 10+ years of clinical experience - required
  • Leadership experience - preferred
  • High proficiency in clinical expertise
  • Strong communication, professionalism, and leadership skills
  • High proficiency in problem-solving related to patient flow, bed management, and transfer coordination
  • Ability to work collaboratively within large teams
  • Proficient in data entry and utilization of EMR and technology
  • Ability to interpret real-time data for informed decision-making
  • Current or prior RN/MD/DO/MBBS/Paramedic/Case Management/Social Work license/certification - required
