Job Summary

The Support Coordinator will work closely with clients to develop and implement individualized care management plans, ensuring seamless coordination of services and supports. This role requires strong communication and interpersonal skills, as well as the ability to navigate complex healthcare systems.

Key Responsibilities

  • Conduct comprehensive assessments to identify client needs and preferences
  • Develop and implement Person-Centered Service Plans, ensuring client choice and autonomy
  • Coordinate with healthcare providers, community resources, and other stakeholders to ensure comprehensive care
  • Monitor and evaluate client outcomes, making adjustments to care plans as needed
  • Ensure confidentiality and maintain accurate records

Requirements

  • BS/BA degree in related field
  • Minimum 3 years experience in case management or related field
  • Strong communication and interpersonal skills
  • Ability to navigate complex healthcare systems
  • Valid driver's license and reliable transportation
