Position Overview

CORA is currently looking for a Family Support Coordinator to join a newly established program within the agency. The primary objective of the Navigation team is to connect children with essential services that alleviate barriers and provide tailored support to families. In this role, you will collaborate with families to assess their child's needs and guide them towards appropriate services. You will work closely with the Navigation team to develop and execute a comprehensive service plan for the identified child, ensuring that support is provided until services are fully established, fostering enduring relationships and support systems.

Work Environment: We prioritize work-life balance and offer flexibility with one remote workday each week, alongside four in-person days at our main office. Additional remote work opportunities may be available following a successful probationary period.

Compensation: $21.66/hour

Salary increases are contingent upon additional qualifications and experience.

Key Responsibilities:

  • Promote the mission, vision, and values of CORA Services, emphasizing respect for all individuals and supporting their potential for development.
  • Assist the Family Navigation Center (FNC) staff in delivering services to clients through administrative and consultative support, including but not limited to:
    • Processing incoming referrals efficiently.
    • Conducting assessments and assigning cases to the Navigation team.
    • Ensuring all FNC Intake documentation is completed accurately.
    • Maintaining and entering client service data in internal systems (EHR) to ensure precise record-keeping.
  • Provide resource and referral assistance to families who may not qualify for CORA programs, including those from outside the county.
  • Develop coverage plans for the intake team during staff absences, collaborating with the IT department as necessary.
  • Support families in establishing and maintaining desired service connections, addressing any barriers to access, including follow-up and assistance with benefits exploration.
  • Adhere to agency guidelines regarding caseload management, service delivery, accountability, supervision, and confidentiality.
  • Maintain regular communication and respond promptly to inquiries.
  • Prepare and submit all required reports and client documentation in a timely manner (EHR and other necessary documentation).
  • Participate in all required supervision and training sessions.
  • Comply with the Social Work Code of Ethics.
  • Provide administrative assistance to the FNC Director and Navigation Supervisor as needed.
  • Manage the FNC Training Hub and RELIAS training systems, registering new employees as required.
  • Perform additional job-related duties as assigned by the supervisor and FNC Director.

Educational Qualifications:

  • A Bachelor's degree in Social Work or a related field; candidates currently enrolled in a relevant degree program with applicable experience in healthcare, behavioral health, or social services may also be considered.

Experience and Skills:

  • A minimum of two years of experience in social services, particularly focused on serving children and families; relevant experience in a hospital or behavioral health setting will be considered.
  • Strong knowledge of community resources, assets, and needs.
  • Understanding of socioeconomic factors affecting children and families.
  • Exceptional interpersonal skills with a client-centered approach.
  • Excellent oral and written communication abilities.
