Job Summary

We are seeking a highly skilled and compassionate Care Manager to join our team at Hearts and Hands of Care, Inc. As a Care Manager, you will play a vital role in ensuring the well-being and quality of life for our clients.

Key Responsibilities
  • Provide case management services to clients, including assessment, planning, and coordination of care.
  • Develop and implement individualized care plans that meet the unique needs of each client.
  • Collaborate with healthcare providers, family members, and other stakeholders to ensure seamless care coordination.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
  • Provide education and support to clients and their families on healthcare-related topics.
Requirements
  • High school diploma or equivalent required; degree in social work, nursing, or related field preferred.
  • Minimum 2 years of experience in case management or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of healthcare systems and regulations.
