About the Role
Fairbanks Memorial Hospital, a non-profit facility owned by the Greater Fairbanks Community Hospital Foundation, is seeking a skilled Social Worker to join their team. As a Social Worker, you will play a vital role in coordinating care and ensuring the smooth transition of patients from acute care to their next level of care.
Key Responsibilities
- Assess and evaluate patient needs, developing and implementing care plans to address social, emotional, and situational stressors.
- Collaborate with the healthcare team to develop, manage, and communicate transition of care plans.
- Document all interventions in the patient medical record, ensuring timely and accurate documentation.
- Provide advocacy and resource services for patients and families, acting as a liaison to healthcare facilities and community groups.
- Maintain knowledge of Medicare, Medicaid, and other program benefits to assist patients with discharge planning and choices.
- Provide professional education to staff and community, staying up-to-date on contemporary behavioral health and system theories.
Requirements
- Bachelor's Degree in Social Work.
- Non-temporary AK LBSW license required within 12 months of hire.
- Temporary LBSW license acceptable within the first year of hire.
- Knowledge of government/community agencies, resources, such as Medicare/Medicaid, long-term care, or other applicable resources/services.
- Effective communication, human relation, and time management skills.
