We are seeking a highly skilled and compassionate Case Manager to join our team at Hearts & Hands of Care, Inc. As a Case Manager, you will play a vital role in ensuring the well-being and quality of life for our consumers.
Key Responsibilities- Provide administrative support to the program team, including data entry, file maintenance, and record-keeping.
- Develop and maintain relationships with consumers, their families, and healthcare providers to ensure seamless care coordination.
- Conduct initial and annual assessments, as well as expedited assessments and amendments, to ensure compliance with state and federal regulations.
- Process new consumer intake documentation, including verification of diagnosis and consumer supplies requests.
- Facilitate consumer meetings, home visits, and fair hearings to ensure program compliance and safety procedures.
- Handle confidential and sensitive information with poise, tact, and diplomacy.
- Perform other duties as assigned, including critical incident reporting and data analysis.
- High school diploma or equivalent required; 2 years of experience in the healthcare field preferred.
- Strong interpersonal skills, customer service, and communication skills.
- Knowledge of HIPAA laws and ability to maintain confidential medical and personal records.
- Proficiency in Microsoft software applications, including Word, Excel, and Outlook.
- Ability to work independently and as part of a team, with a high level of poise, tact, and diplomacy.
- Office setting with prolonged sitting.
- Extended hours, including evenings and weekends, may be required.
- Travel to and from clients' residences may be necessary.
