Job Summary

As a Transplant Nurse Coordinator at Hartford Hospital, you will play a critical role in the multidisciplinary clinical care of potential and actual transplant candidates and recipients. This position requires a skilled and compassionate nurse who can coordinate the clinical management of pre or post-transplant patients, manage data for transplant patients, and develop educational programs to meet the needs of nurses, medical personnel, patients, families, and the community.

Responsibilities
  • Pre-Transplant Coordination: Conduct pre-transplant interviews, coordinate pre-transplant evaluations, communicate with referring physicians, and present patients to the transplant team to ensure appropriateness for transplantation.
  • Transplant Health Care Team Collaboration: Participate in establishing and accomplishing goals, function as a change agent by consulting and communicating with caregivers across clinical areas to improve clinical outcomes and cost-effectiveness of transplantation patient care.
  • Patient Education and Support: Identify educational needs of patients, families, populations, healthcare providers, and students, and develop programs, protocols, and policies to meet those needs.
  • Research and Quality Improvement: Participate in research studies, interpret, disseminate, and utilize findings in practice to improve quality, cost, and outcomes of patient care, and mentor and coach staff in conducting research and utilizing findings.
  • Organizational Representation: Represent the organization at local, regional, and national levels through speaking, presenting, and publishing to identify the organization as a center of excellence.
  • Competencies and Quality Patient Service: Demonstrate competencies, including knowledge and behaviors, to meet age-specific patient needs for the heart transplant population or for the management of cadaver organ donors, and model quality patient/customer service by responding to patient needs and maintaining confidentiality of patient information.
Qualifications
  • Current Connecticut registered Nurse License (BSN Required)
  • 2-4 years recent clinical acute care experience
  • Experience in transplantation and/or dialysis preferred
  • Certification or eligibility within specialty may be required
