About the Role
Hartford Healthcare (HHC) is seeking a highly skilled and dedicated Nurse Practitioner to join our esteemed Transplant Clinic team. As a key member of our multidisciplinary team, you will provide comprehensive medical care to patients who have undergone kidney or other solid organ transplants.
Key Responsibilities
- Conduct outpatient clinic visits for post-transplant patients, including taking medical histories, performing physical examinations, and ordering diagnostic tests.
- Manage post-transplant care, including monitoring patient progress, managing immunosuppressive therapy, and coordinating follow-up care.
- Collaborate with transplant surgeons, nephrologists, nurses, social workers, and other healthcare professionals to ensure comprehensive patient care.
- Coordinate follow-up care, including scheduling appointments, managing referrals, and communicating with other specialists.
- Maintain accurate and up-to-date patient records, including progress notes, treatment plans, and communication with other healthcare providers.
Requirements
- Unrestricted State of Connecticut Medical License (or eligible)
- APRN (Family, Pediatric, Adult/Geri Nurse Practitioner) or PA training
- At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA) (preferred)
