Job Summary

As a key member of the Hartford Hospital team, the Pre-Kidney Coordinator plays a vital role in providing and coordinating multidisciplinary clinical care for potential and actual transplant candidates and recipients.

Responsibilities
  • Pre-Transplant Coordination: Conducts pre-transplant interviews, coordinates pre-transplant evaluations, and communicates with referring physicians to ensure patient appropriateness for transplantation.
  • Transplant Health Care Team Collaboration: Participates in establishing and accomplishing goals, functions as a change agent by consulting and communicating with care-givers across clinical areas to improve clinical outcomes and cost-effectiveness of transplantation patient care.
  • Patient Education and Support: Identifies educational needs of patients, families, and populations, and develops programs, protocols, and policies to meet those needs.
  • Transplant Rounds and Patient Visits: Attends transplant rounds, conducts pre-transplant visits to potential inpatients, and conducts follow-up visits to patients after transplantation to assess and evaluate patient needs.
  • Research and Quality Improvement: Participates in research studies, interprets, disseminates, and utilizes findings in practice to improve quality, cost, and outcomes of patient care.
  • Organizational Representation: Represents Hartford Hospital at local, regional, and national levels through speaking, presenting, and publishing to identify the organization as a center of excellence.
  • Competencies and Quality Patient Service: Demonstrates competencies, including knowledge and behaviors, to meet age-specific patient needs for the heart transplant population or for the management of cadaver organ donors.
  • Care Improvement and Staff Development: Contributes to care improvement by coaching, developing, and mentoring staff, participating in performance/quality improvement opportunities, and research.
Qualifications
  • Current Connecticut registered Nurse License (BSN Required)
  • 2-4 years recent clinical acute care experience
  • Experience in transplantation and/or dialysis preferred
  • Certification or eligibility within specialty may be required
