Job Overview:

The primary objective of the Home Health Registered Nurse is to oversee and manage all facets of patient care, including the supervision of Licensed Vocational Nurses (LVNs) and Home Health Aides (HHAs), as well as coordinating case discussions and discharge strategies to address the medical, social, and emotional needs of patients.

This role emphasizes enhancing patient outcomes during their home health journey and working collaboratively with physicians, clinical teams, and patients to ensure a smooth transition to discharge.


Key Responsibilities:
  • Oversee and direct all patient care activities, including the supervision of LVNs and HHAs.
  • Organize the allocation of visits among various healthcare disciplines (Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work, Home Health Aide) following the initiation of care, recertification, or resumption of care, and coordinate the plan of care with the physician.
  • Initiate referrals to necessary ancillary services such as podiatry and community resources.
  • Facilitate communication among interdisciplinary teams to ensure clarity regarding patient needs and expectations for service delivery.
  • Lead weekly interdisciplinary case discussions for assigned patients and contribute to the formulation of individualized care plans.
  • Develop specific social action plans and achievable goals for each patient, aligned with their medically related social and emotional requirements.
  • Collaborate with the Administrator to interpret and advocate for patients' rights.
  • Manage all aspects of discharge planning in conjunction with patients, clinical teams, and the agency.
  • Provide individual and group support for patients and families during times of transition, crisis, or specific needs.
  • Complete all required documentation, including assessments, evaluations, visit notes, and discharge summaries, in compliance with state and federal regulations.
  • Act as a team leader and role model within the organization.
  • Participate in various committees as requested by the Administrator.
  • Familiarize with and uphold the Clinical Policies and Procedures on Abuse Prevention.
  • Ensure patient safety at all times.
  • Perform additional duties as assigned.

Registered Nurse Essential Duties:
  • Communicate all Start of Care, Resumption of Care, and recertification details to the team via secure messaging or email, including discipline orders and relevant information for care provision.
  • Complete all OASIS documentation by 9:00 AM the following day, seven days a week.
  • Participate in the on-call weekend rotation as required, including for PRN staff, with frequency determined by branch size.
  • Full-time staff may be expected to work at least one major and one minor holiday annually, with additional holiday coverage as needed based on branch size and staffing levels.
  • PRN staff may be required to work one major and one minor holiday each year, at least two weekend days monthly, and complete a minimum of five visits monthly unless on an approved leave of absence.
