Key Responsibilities- Visit patients in their homes and provide nursing care under the direction of RNs and in compliance with physicians' orders.
- Assess patients' physical and emotional needs, and develop individualized care plans to meet those needs.
- Administer medications, perform wound care, and provide other nursing interventions as needed.
- Collaborate with RNs, therapists, and other healthcare professionals to ensure comprehensive care.
- Document patient progress and communicate effectively with patients, families, and healthcare teams.
- Current licensure as a Licensed Vocational Nurse in the state of employment.
- Valid state driver's license and automobile liability insurance.
- Dependable transportation and ability to drive in all types of weather conditions.
