Job Summary

We are seeking a skilled Licensed Vocational Nurse (LVN) to join our team at Enhabit Home Health & Hospice. As a LVN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Visit patients in their homes and provide nursing care under the direction of RNs and in compliance with physicians' orders.
  • Assess patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Administer medications, perform wound care, and provide other nursing interventions as needed.
  • Collaborate with RNs, therapists, and other healthcare professionals to ensure comprehensive care.
  • Document patient progress and communicate effectively with patients, families, and healthcare teams.
Requirements
  • Current licensure as a Licensed Vocational Nurse in the state of employment.
  • Valid state driver's license and automobile liability insurance.
  • Dependable transportation and ability to drive in all types of weather conditions.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
