Healthcare Coordinator - LVN

We are seeking a skilled and compassionate LVN to join our team as a Healthcare Coordinator. In this role, you will play a critical part in supporting patients with high-risk health conditions navigate the healthcare system.

Key Responsibilities:

  • Work with providers and clinic staff to identify patients at high risk through transitions of care.
  • Support longitudinal care of patients with chronic care conditions by performing assessments, initiating medication reconciliation, and providing patient education.
  • Support transition of patients with chronic care conditions from inpatient to outpatient settings by performing assessments, initiating medication reconciliation, and establishing contingency plans.
  • Coordinate with providers to establish or update plans of care.
  • Perform accurate and timely documentation in the electronic medical record.

Requirements:

  • Licensed Practical/Vocational Nurse with an active and unrestricted license to practice in the state of employment.
  • Current BLS certification.
  • 2+ years of experience in a physician's office, clinical, or hospital setting.
  • Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD.
  • Proficient computer skills to work efficiently with electronic medical records.
  • Proven ability to interact productively with individuals and multidisciplinary teams.
  • Proven excellent verbal and written skills.
  • Proven solid interpersonal skills.
  • Proven excellent organizational and prioritization skills.
