Client Intake and Assessment Coordinator (Bilingual Preferred)

Work Environment

This position offers a hybrid work model, allowing you to engage with clients in a professional setting while also providing the flexibility to work remotely.

Key Responsibilities

The Client Intake Coordinator serves as the initial point of contact for prospective clients seeking Early Start and ongoing services. This role involves gathering essential information to assess eligibility and provide insight into available services. Travel to various locations for meetings and client interactions is required. Proficiency in Spanish or Cantonese, both written and verbal, is highly valued.

  • Client Engagement:
    • Collect comprehensive intake information from applicants for GGRC services and refer qualified individuals to the Assessment Team.
    • Document intake details and notes in the system.
    • Verify the client's initial status within the Regional Center system.
    • Offer general information and referrals regarding resources and services to clients and callers.
    • Clarify GGRC procedures and policies, including relevant legislation and rights.
    • Direct clients to additional community resources as needed.
    • Ensure compliance with required timelines and organizational policies.
  • Data Management:
    • Maintain a database of inquiries and document all applicant interactions.
    • Complete and uphold all necessary forms, documentation, and reports in accordance with regulations.
    • Assess residency status based on established guidelines.
    • Evaluate the suitability of service applications.
  • Administrative Support:
    • Manage information and referral services.
    • Compile monthly statistics.
    • Assist in identifying referrals for outreach initiatives.
    • Provide administrative assistance to the unit as required.
  • Participate in special projects and undertake additional responsibilities as assigned.

All client information must be treated with confidentiality in accordance with the federal Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the California Lanterman Act.

Qualifications

Education and Experience:

  • Bachelor's Degree in a human services field, preferably in social work.
  • Minimum of 1 year of experience in client interaction and evaluation.
  • Experience working with individuals with developmental disabilities.
  • Background in Early Childhood Development is desirable.
  • Case Management experience is a plus.
  • Experience in multicultural environments and/or multilingual capabilities is preferred.

Skills and Competencies:

  • Ability to communicate clearly with native Spanish or Cantonese speakers and comprehend relevant dialects.
  • Strong interviewing skills with the capacity to assess individual needs and accurately document details.
  • Excellent written and verbal communication skills, capable of producing professional documents and conveying complex information succinctly.
  • Familiarity with issues related to developmental disabilities.
  • Proficient in MS Office and Adobe software.
  • Competent in operating standard office equipment.
  • Strong time management and organizational abilities.
  • Proactive decision-making skills.
  • Detail-oriented with a customer-focused approach.
