Job OverviewPosition Summary:

As a Sales and Relationship Manager at Pathwell Health, you will play a pivotal role in driving business growth by fostering connections with patients, families, and key stakeholders such as healthcare professionals and case managers. Your efforts will ensure seamless care transitions and effective communication across various healthcare settings.

Key Responsibilities:
  • Develop and sustain productive partnerships with case managers, discharge planners, and other healthcare professionals across various facilities.
  • Conduct impactful sales meetings and calls that identify and address the needs of referral partners, including thorough pre-and post-call planning and rapport building.
  • Provide education to the medical community regarding home health services through effective communication and resource distribution.
  • Meet and exceed weekly, monthly, and personal performance targets by nurturing both existing and potential referral sources.
  • Maintain accurate documentation, engage in regular team meetings, and manage essential paperwork efficiently.
  • Collaborate in interdisciplinary team discussions and marketing strategy sessions.
  • Support the Intake Coordinator in ensuring timely patient admissions and care coordination.
  • Address and resolve any concerns or dissatisfaction expressed by referral sources or patients/families.
  • Adhere to all organizational policies and procedures while performing related duties as assigned.

Qualifications (Knowledge, Skills, and Abilities):
  • Associate Degree or equivalent experience is required.
  • Prior experience in Home Health or Hospice is preferred.
  • Familiarity with medical terminology is essential.
  • Exceptional written and verbal communication skills are necessary.
  • Strong customer service orientation and relationship-building capabilities are vital.
  • Proficiency in CRM software, Google Workspace, and Microsoft Office Suite is required.
  • A valid driver's license and a car with current insurance coverage are mandatory.
  • Willingness to travel to various client and community locations on short notice.

Skills & Competencies:
  • Tech-savvy and highly organized individual.
  • Understanding of the post-acute care ecosystem and referral processes.
  • Ability to interpret medical terminology to evaluate referral needs effectively.
  • Dedicated and diligent, capable of managing a high volume of in-person visits weekly.
  • Existing connections with healthcare facilities and professionals are advantageous.
  • Sales-oriented mindset with experience in using CRM tools for organization.
  • Coordinate marketing efforts with the sales operations team.

Physical Requirements:
  • Ability to lift and carry up to 25 lbs.
  • Capability to navigate various healthcare settings and travel as needed.
  • Comfortable standing or sitting for extended periods.
  • Proficient in using communication tools for prolonged durations.
