Job Overview
Baylor Scott & White Healthcare is seeking a skilled Member Advocate to join our team. As a Member Advocate, you will play a critical role in navigating members through the healthcare system, overcoming non-clinical obstacles, and ensuring timely delivery of quality care.

About the Role
This position requires a highly motivated individual with excellent communication skills, able to work autonomously and collaboratively with multiple teams. The successful candidate will have experience as a community health advocate and/or navigator, with knowledge of the environment, healthcare system, and community resources.

Responsibilities
- Establish trusting relationships with members, assess barriers to care, and serve as an advocate in overcoming identified barriers
- Conduct member screenings, refer to case management if clinical needs are identified, and maintain documentation of member contact and referrals
- Build partnerships with teams across departments, provider offices, and community resources to foster timely delivery of quality care

Requirements
- Associate's or 2 years of work experience above the minimum qualification
- 3 Years of Experience as a Community Health Advocate and/or Navigator
