Care Navigator Specialist

Integrative Emergency Services is seeking a dedicated and compassionate Care Coordinator to join our team. As a key member of our staff, you will be responsible for providing exceptional patient care and experience.
Job SummaryThe successful candidate will perform administrative and clinical duties under the direction of a licensed healthcare provider. This includes facilitating communication with patients and providers, scheduling routine tele-visits, and assisting with acquiring patient records. Additionally, you will be responsible for creating and maintaining care plans based on provider-approved protocols and ensuring compliance with CMS guidelines.
Key Responsibilities:- Review assigned patients to make timely and appropriate patient care calls.
- Schedule patients for routine tele-visits with providers.
- Communicate with providers, including outside specialists involved in patient care.
- Assist in acquiring patient records.
- Create and maintain care plans based on provider-approved care plans for patient-centered care.
- Ensure care plans are shared with patients and families based on CMS guidelines.
- Provide patient and family education.
- Ensure compliance with CMS guidelines for Chronic Care Management.
- Assist in enrolling new patients to the Guided Care+ program.
- Communicate with interdisciplinary teams, such as social workers and home health for patient needs.
- Report errors or issues promptly to supervisor.
- Maintain security of medical records and patient's health information according to HIPAA standards.
- Communicate with providers regarding immediate concerns with patients.
- Follow through with patient inquiries, requests, and complaints. Forward difficult and non-routine inquiries or requests to appropriate level for resolution.
- Provide occasional work guidance, technical advice, and training to staff.
You must have 2+ years of experience in health education, patient navigation, and/or health clinic patient assistance. A High School Diploma or GED is required, along with a valid Texas Driver's license and reliable transportation. Preferred qualifications include certification for Medical Assisting, an active nursing license on the Texas Board of Nursing, and bilingualism (Vietnamese, Chinese, Arabic, and/or Spanish with English).
