We are seeking a Pediatric Care Navigator to join our team at Children's Health (Dallas). This is a full-time position with an estimated salary of $85,000-$110,000 per year.

About the Role

The Pediatric Care Navigator will be responsible for coordinating care throughout the continuum of care for an assigned patient population. They will work in partnership with patients, families, and other caregivers to facilitate seamless transitions from one level of care to another.

Responsibilities
  • Coordinate care across the continuum, ensuring continuity, quality, and closure of gaps in care
  • Advocate for patient needs and coordinate with multidisciplinary teams to ensure effective resource management
  • Develop and implement strategies to maximize the most appropriate use of services
  • Work closely with payors, suppliers, and equipment providers to ensure timely access to necessary resources
  • Collaborate with healthcare teams to develop and implement plans of care that promote best outcomes
Requirements
  • At least 4 years of experience in pediatric nursing, case management, care coordination, or related fields
  • Registered Nurse in the State of Texas
  • Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification preferred
  • Excellent communication and interpersonal skills
  • Able to work independently with minimal supervision
