Care Transition Navigator

Estimated Salary: $65,000 - $85,000 per year

Methodist Charlton Medical Center, a 314-bed acute care teaching hospital, seeks a Care Transition Navigator to coordinate activities promoting quality outcomes and patient throughput.

The ideal candidate will have a Bachelor's or Master's degree in Social Work or a related field, with at least one year of experience in a healthcare setting. Registered Nurses with a BSN are preferred.

This role requires excellent communication and relationship-building skills, as well as accountability and a commitment to continuous improvement.

Methodist Charlton Medical Center is a Magnet-designated hospital and has been recognized for excellence in cardiac catheterization, electrophysiology, and STEMI care.
