Job Description

Spectraforce Technologies seeks a highly skilled Care Navigator Specialist to join our team. This role involves working with patients, healthcare providers, and community resources to ensure seamless care delivery.

Key Responsibilities:

  • Conduct telehealth assessments and in-person visits as needed
  • Review patient assessments and service plans to identify areas for improvement
  • Follow up on pending assessment findings and clarify diagnosis, abnormal results, and referrals
  • Coordinate hospital discharges and follow up on denials
  • Research community resources and provide updates to patients and managers
  • Update LTSS records and terminate waiver service authorizations as necessary
  • Provide fair hearing assistance and determine MN for post-assessment identified items

Requirements:

  • BS in Social Work or related field
  • Previous MCO or IDD waiver experience preferred
  • Excellent computer skills and proficiency in Microsoft Word, Outlook, and Excel
  • Bilingual preferred

Salary: $60,000 - $80,000 per year, depending on experience
