Position Title: Nurse Navigator

Location: Wilmington, DE

Schedule: Monday - Friday

POSITION SUMMARY

In this pivotal role, the Nurse Navigator operates under the guidance of the Chief Medical Officer, overseeing the Care Coordinator Program. This position is essential in fostering high standards of professional conduct and performance, while also taking proactive measures to enhance patient care.

KEY RESPONSIBILITIES

  • Facilitate timely access to necessary healthcare services.
  • Encourage the use of preventive healthcare measures.
  • Minimize emergency room visits and hospital readmissions.
  • Enhance patient understanding through culturally sensitive education.
  • Develop and promote adherence to personalized care plans in collaboration with patients and their primary care providers.
  • Ensure continuity of care by managing relationships with specialty providers and overseeing transitions in care.
  • Empower patients in self-management and shared decision-making processes.
  • Conduct medication reconciliation for patients.
  • Connect patients with appropriate community resources to improve health outcomes and satisfaction.
  • Perform additional duties as required.

PERFORMANCE EXPECTATIONS

  • Support the integration of primary and specialty care through effective communication and follow-up regarding patient transitions and chronic disease management.
  • Identify and manage high-risk patients, ensuring they are appropriately flagged in the patient registry.
  • Provide constructive feedback to enhance the Care Coordination Program and manage patient interactions effectively.

QUALIFICATIONS

  • Must possess a valid and accredited Registered Nurse license.
  • 3-5 years of experience in a clinical or community health environment.
