POSITION SUMMARY

In this pivotal role, the Nurse Navigator operates under the guidance of the Chief Medical Officer, overseeing the Care Coordinator Program. This position demands a proactive approach and a commitment to maintaining high standards of professionalism and performance.

KEY RESPONSIBILITIES

  • Facilitate timely access to suitable healthcare services.
  • Enhance the use of preventive health measures.
  • Minimize emergency department visits and hospital readmissions.
  • Improve patient understanding through culturally sensitive education.
  • Develop and uphold a care plan in collaboration with patients and their primary care providers.
  • Strengthen continuity of care by managing relationships with specialized care providers and overseeing transitions in care.
  • Empower patients in self-management and collaborative decision-making.
  • Conduct medication reconciliation.
  • Link patients to pertinent community resources to boost health outcomes and satisfaction.
  • Perform additional tasks as required.

PERFORMANCE EXPECTATIONS

  • Oversee the development and support of co-management between primary and specialty care providers, ensuring effective communication and integration of information into care plans.
  • Identify and monitor 'high-risk' patients, including those with chronic illnesses, and maintain an updated patient registry.
  • Provide constructive feedback to enhance the Care Coordination Program and manage patient interactions effectively.

QUALIFICATIONS

  • Current licensure and credentials as a Registered Nurse.
  • A minimum of 3-5 years of experience in clinical or community health environments.
