Overview:
The Clinical Nurse Coordinator (CNC) for the Community Living Center (CLC) is tasked with delivering proficient, evidence-based healthcare to assigned residents while overseeing the activities of licensed vocational nurses and nursing assistants as appropriate to the environment.

Key Responsibilities:
This role qualifies for the Education Debt Reduction Program (EDRP), which offers student loan repayment assistance. Applicants must fulfill specific eligibility criteria in line with VHA policy and submit their EDRP application within the designated timeframe after appointment.

The CNC will lead nursing education initiatives, orientation, competency assessments, and provide consultation on quality improvement and outcome utilization.

Demonstrating leadership in enhancing holistic care, the CNC will collaborate with interdisciplinary teams to implement effective strategies.

The role includes managing the CLC Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) program, acting as the Facility Administrator for the RAI/MDS program and its software, and advocating for the effective use of RAI/MDS assessments in resident care planning.

Additionally, the CNC will supervise the RAI/MDS RN responsible for coding nursing sections and Care Area Assessments (CAAs), and will identify team members for conducting quarterly interrater reliability reviews.

Collaboration with CLC management or charge nurses is essential to support the interdisciplinary team in conducting these reviews, which evaluate the accuracy of RAI/MDS assessments and report error trends.

The CNC will complete and submit quarterly interrater reliability reports to the CLC leadership team. In the absence of a CLC MDS RN, the CNC is responsible for documenting and addressing any discrepancies identified during these reviews.

Education of CLC staff on the RAI/MDS process is vital to ensure proper completion.

As an expert consultant, the CNC will identify quality of care deficiencies and assist with performance improvement initiatives related to quality management.

The CNC will also manage the downloading, validation, and distribution of quality management and Resource Utilization Group (RUG) reports to the CLC interdisciplinary team and leadership.

Preparation for ongoing survey readiness for annual unannounced surveys and Interim Quality Oversight surveys is a critical aspect of this role, ensuring compliance with regulatory guidelines for scheduling assessments.

Work Schedule:
Standard hours are from 7:30 AM to 4:00 PM. Telework and relocation options are not applicable.

Not required. EDRP is authorized, and inquiries can be directed to the EDRP Coordinator.

Conditions of Employment:
U.S. Citizenship is required; non-citizens may only be appointed when qualified citizens are unavailable. All applicants selected for VA employment in designated testing positions must undergo urinalysis for illegal drug use prior to appointment. Selective Service Registration is mandatory for males born after December 31, 1959. Background/security investigations will be conducted, and selected candidates must complete an online onboarding process and pass a pre-employment physical examination.

Participation in the seasonal influenza vaccination program is required for all Department of Veterans Affairs Health Care Personnel (HCP), as is participation in the COVID-19 vaccination program.

Qualifications:
Basic requirements include proficiency in English, graduation from an accredited nursing program, and current, unrestricted registration as a professional nurse in a U.S. state or territory.

Preferred Experience:
A Bachelor of Science in Nursing (BSN) and experience in MDS coordination are preferred.

Physical Requirements:
This position requires the ability to perform various physical tasks, including prolonged walking, standing, and manual dexterity. Exposure to potentially infectious patients and materials may occur, necessitating the use of protective clothing.
