CLINICAL SOCIAL SERVICES SPECIALIST

Full Time, Days

Position Overview:

  • Acts as a Social Services Specialist for the organization.
  • Facilitates patient transitions in alignment with established protocols and regulatory standards.
  • Ensures that necessary components of the Minimum Data Set (MDS) are accurately completed as required.
  • Guarantees that a significant social program tailored to the individual needs of each patient is implemented.
  • Directs all residents or families to suitable agencies or resources when services cannot be provided internally.
  • Collaborates closely with Unit Directors and RN Supervisors, as well as residents and/or family members, to gather insights regarding admissions, discharges, and room assignments.
  • Supports the onboarding of new residents, including the completion of relevant documentation and adjustment to their new surroundings.
  • Assists families or residents with placement and discharge information as necessary.

Qualifications:

  • Bachelor's degree and a minimum of one year of experience in the healthcare sector or a comparable combination of education and experience.
  • Familiarity with local, state, and federal regulations pertaining to the social needs of patients.
  • Experience in admission and/or discharge planning is preferred.
