Key Responsibilities:

  • Case Management: Coordinate the delivery of patient care services, ensuring that patients receive high-quality, compassionate care.
  • Staff Supervision: Supervise and mentor assigned care staff, providing guidance and support to ensure that they are equipped to provide excellent patient care.
  • Documentation: Maintain accurate and timely documentation of patient care and staff performance.
  • Collaboration: Work closely with the administrator to ensure that the hospice program is operating efficiently and effectively.
  • Quality Improvement: Identify areas for quality improvement and implement changes to enhance patient care and staff performance.

Requirements:

  • Education: Current RN licensure in North Carolina and successful completion of nursing education from an approved school of nursing.
  • Experience: Minimum three (3) years of clinical nursing experience, with a focus on hospice care.
  • Skills: Strong critical thinking and communication skills, with the ability to independently organize work and provide effective case management.
  • Certifications: CPR certification.
