Job Summary:

The Director of Social Services is a key member of the Nexion Health team, responsible for providing medically related social services to residents. This role assumes administrative authority and accountability for the provision of social services that assist residents to attain or maintain the highest practicable physical, mental, and psycho-social well-being.

Responsibilities:

  • Meet with residents and families to review policies on advanced directives, resident rights, and discrimination.
  • Conduct and document social services evaluations, including identification of resident problems/needs.
  • Provide and arrange for social work services as indicated by resident/family needs.
  • Assist residents with financial planning, as needed.
  • Keep residents' families informed of resident problems, personal needs, transfers, and changes of level assignment.
  • Maintain a current list of community resources to facilitate referrals.
  • Establish criteria to ensure that services provided meet established standards of quality.
  • Demonstrate satisfactory interpersonal skills to interact with residents, families, community, and interdisciplinary staff.
  • Evaluate residents' behavior, collect data to evaluate psycho-social needs, risk factors for psycho-social deterioration, and residents' responses to interventions.
  • Implement social service interventions that achieve treatment goals, address resident needs, link social supports, physical care, and physical environment to enhance quality of life.
  • Communicate effectively in an interdisciplinary care setting with residents, families, staff members.
  • Carry out all duties in accordance with the facility mission and philosophy.
  • Demonstrate knowledge of and respect for the rights, dignity, and individuality of each resident in all interactions.
  • Maintain confidentiality of resident and facility information.
  • Demonstrate honesty and integrity at all times in the care and use of resident and facility property.
  • Knowledge of emergency and disaster procedures of the facility.
  • Ability to prioritize tasks/responsibilities and complete duties/projects within allotted time.
  • Ensure that the quality and appropriateness of resident care meets or exceeds company and industry standards.
  • Maintain effective relationships and open communication with residents, families, staff.
  • Participate in quality improvement activities to reduce cost and increase efficiency through improved systems and processes.

Requirements:

  • Minimum of Bachelor's Degree in Social Work or in a human services field (including but not limited to Sociology, Special Education Rehabilitation Counseling, Psychology).
  • One year supervised social work experience working directly with individuals in a healthcare setting.
  • Current Social Work license in the state.
