KEY RESPONSIBILITIES:

  • Evaluate patients' comprehension and provide education on the relationship between nutrition, kidney health, and treatment outcomes.
  • Inform patients, families, and caregivers about nutritional status, fluid management, and relevant laboratory results related to their health conditions.
  • Conduct comprehensive nutritional evaluations and collaborate with the healthcare team to create individualized care plans for patients.
  • Maintain precise medical documentation, including progress notes.
  • Provide continuous assessment and guidance to assist patients in achieving optimal nutritional health.
  • Identify individuals at risk of malnutrition and coordinate interventions with the healthcare team.
  • Monitor and modify nutrition therapy, documenting progress and addressing any challenges.
  • Collaborate with the team on Urea Kinetic Modeling recommendations.
  • Engage in nutrition Quality Assessment & Improvement (QAI) initiatives.
  • Analyze and interpret nutrition-related laboratory results.
  • Work in conjunction with physicians and facility staff as necessary.
  • Maintain dietetic registration and meet continuing education requirements.

EDUCATIONAL REQUIREMENTS:

  • Registered Dietitian accredited by the Commission on Dietetic Registration.
  • Board Certified Specialist in Renal Nutrition certification is advantageous.
  • Valid state licensure as required.

EXPERIENCE AND QUALIFICATIONS:

  • Minimum of 1 year of clinical nutrition experience as a Registered Dietitian.
  • Previous experience in renal nutrition is preferred.

WORKING CONDITIONS:

  • The role involves desk work, computer usage, and interaction with patients, staff, and physicians. Travel between facilities and community locations may be required. Occasional travel for meetings may also be necessary.
  • The work environment resembles a healthcare facility, with controlled temperatures and moderate noise levels. There may be potential exposure to infectious materials.
