Stroke Care Program Coordinator

Primary Function/General Purpose of Position

The Stroke Care Program Coordinator is responsible for ensuring the quality, timeliness, and effectiveness of neuroscience care delivery. This role encompasses professional program activities, assessment, planning, organization, implementation, performance improvement, education, utilization of stroke program hospital services, community outreach, pre-hospital education, and marketing/communications. The Coordinator works collaboratively across disciplines to promote excellence in clinical practice for a designated patient population, aiming to provide superior quality, competitive value, and outstanding service to patients, families, and the community.

Additionally, this position aids in assessing, planning, coordinating, implementing, and evaluating regional emergency preparedness initiatives.

The Coordinator is tasked with planning, organizing, and evaluating pre-hospital care activities for the facility. Acting as a liaison between pre-hospital care providers, EMS students, and emergency department staff, the Coordinator represents the emergency department on various internal and external committees related to pre-hospital care. This role promotes high-quality pre-hospital and emergency care by serving as a role model and resource for pre-hospital and emergency department personnel. The Coordinator also coordinates educational activities for the EMS community and stays informed about EMS protocols, policies, and regulations.

Licensing/Certification

EMT-P, EMT-Paramedic, or Registered Nurse

ACLS, PALS, BLS

Preferred: Certified Healthcare Safety Professional (CHSP)/Certified Healthcare Emergency Professional (CHEP)

Instructor status is a plus

Education

2 years/Associates Degree in Emergency Management/Law Enforcement, Vocational/Technical Degree in Fire Service, or Accredited Nursing School

Work Experience

Preferred exposure to critical care and/or acute care of stroke.

Field paramedic experience (5 years preferred).

Registered Nurse with critical care experience (2 years preferred).

Training

Strong collaborative and interpersonal skills to drive program improvement and relationships.

Ability to multi-task and manage multiple projects in a fast-paced environment.

Completion of NIMS courses, FRAME course, HERT/TERT training required.

Proficient with various computer applications.

Essential Job Functions
  • Maintains open communication with EMS agencies, serving as the conduit between the emergency department and EMS providers.
  • Provides EMS education, including scheduling for paramedic students.
  • Represents the organization at regional EMS Coordinator meetings.
  • Regularly visits EMS agencies within the service area.
  • Plans and executes activities for EMS week and ensures EMS resources are adequately supplied.
  • Solicits and responds promptly to EMS follow-ups.
  • Maintains and expands current knowledge related to EMS practices.
  • Responsible for follow-up on EMS bloodborne exposures.
  • Collaborates closely with emergency department staff to resolve issues between the department and EMS.
  • Assists with emergency preparedness efforts, including policy/procedure development, training, report writing, and participation in local, state, and regional meetings.
  • Provides input to management and staff on Emergency Management issues.
  • Serves as a role model and resource person.
  • Chairs or serves on committees as assigned.
  • Identifies and reports potential safety issues.
  • Demonstrates teaching and classroom management skills.
  • Supports and contributes to organizational cost-saving initiatives.
  • Maintains effective communication.
  • Keeps current with neuroscience patient care and collaborates with nursing leadership to uphold quality standards of neuro/stroke care.
  • Coordinates all stroke-specific survey or accreditation efforts with the goal of achieving Primary Stroke Center status.
  • Manages all data activities, including Stroke Registry data entry and neuro/stroke care monitoring.
  • Evaluates the relevance of current research-based knowledge to identify practice problems, alternative solutions, or proposed innovations.
  • Develops, coordinates, and maintains stroke program policies and procedures.
  • Co-Chairs the Stroke Committee and actively participates in system-wide stroke projects and initiatives.
  • In collaboration with Nursing Leadership and the Medical Director, coordinates Stroke Program Performance Improvement activities and ensures integration with hospital and medical staff performance improvement processes.
  • Participates in the development, approval, and implementation of neuro/stroke-related nursing standards, policies, and procedures, particularly regarding care delivery methods, quality monitoring processes, and nursing practice evaluation.
  • Works to foster collaboration across the continuum of stroke care, including EMS/life flight, emergency care, inpatient services, post-acute services, and outpatient options.
  • Encourages associates to engage in community benefit activities, including coordinating and providing stroke community education.
  • Facilitates Program CME records for all Medical Staff and Program members.
  • Conducts and facilitates, at minimum, quarterly seminars and educational activities for physicians, nursing, EMS, and ancillary departments involved in neuroscience patient care, as well as community and media.
