Job Responsibilities:

The Intake Referral Coordinator is responsible for ensuring excellent customer service by managing patient referrals for home care services. The ideal candidate will possess prior experience in home health care or intake processes. Key responsibilities include:

  • Providing a positive and professional phone presence as the first point of contact for referral sources and patients.
  • Answering calls for intake information and accurately entering data into the EMR system.
  • Completing intake forms, coding with ICD-10, and routing for admission approval.
  • Conducting initial nursing insurance authorizations for new referrals and maintaining a tracking log.
  • Keeping a weekly referral log for all intakes.
  • Communicating complex cases to the Intake Manager for further evaluation.
  • Assisting with training clinical staff when educational opportunities arise.
  • Performing LPN duties as needed in case of uncovered visits.
  • Building positive relationships with staff and community resources.
  • Assisting in other branches as required.
  • Carrying out additional duties as assigned by the Intake Manager or Administrator.

Pay: $25.00 per hour

Standard Shift: Day shift

Weekly Schedule: 5x8
