Hospice RN Case Manager Position

At Interim HealthCare Staffing, we are seeking a compassionate and skilled Registered Nurse to fill the role of Hospice RN Case Manager. This position offers a unique opportunity to make a meaningful difference in the lives of patients and their families.
Key Responsibilities:
- Maintain a specified case management load to ensure seamless care delivery.
- Conduct comprehensive assessments of patients and their families to identify hospice needs, including pain and symptom management.
- Participate in on-call duties as defined by the office on-call policy to provide 24/7 support.
- Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families.
- Coordinate care among IDG members and external healthcare providers to ensure continuous assessment and implementation of the IDG plan of care.
- Evaluate and coordinate the need for services provided to patients and families in accordance with the IDG plan of care.
- Supervise and direct the services provided by LPN/LVN, Hospice Aide, and Homemaker assigned to patients and families.
- Facilitate consideration of alternative levels of care when indicated by patients' and families' status and needs.
Requirements:
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in the state of South Carolina with an active and good standing license.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
- One (1) year of hospice care experience.
- One (1) year of precepting or training experience preferred.
- CPR certification.
- Able to provide proof of valid driver's license and auto liability insurance.
