Hospice RN Case Manager Position

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a key member of our interdisciplinary team, you will provide exceptional care to patients and families, upholding our high standards of nursing practice.
Key Responsibilities:
- Maintain a specified case management load, ensuring timely and effective care coordination.
- Conduct comprehensive assessments of patients and families, identifying their unique needs and developing personalized plans of care.
- Collaborate with the Interdisciplinary Group (IDG) to ensure seamless care delivery and achieve the goals of hospice care.
- Coordinate services among IDG members and external healthcare providers, ensuring continuous assessment and implementation of care plans.
- Supervise and direct the services provided by LPN/LVN, Hospice Aide, and Homemaker assigned to patients and families.
Requirements:
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
- One (1) year of hospice care experience, with precepting or training experience preferred.
- CPR certification and valid driver's license with auto liability insurance.
