We are seeking a compassionate and skilled Hospice RN Case Manager to join our team at Interim HealthCare Staffing. As a Hospice RN Case Manager, you will play a vital role in providing exceptional care to patients and families in Charleston, SC.
Key Responsibilities:
- Maintain a specified case management load.
- Conduct initial and ongoing assessments of patients' physical and psychosocial status.
- Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG).
- Coordinate care among IDG members and other healthcare providers to ensure seamless delivery of services.
- Evaluate and coordinate the need for services provided to patients and families.
Requirements:
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in the state of South Carolina.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years.
- One (1) year of hospice care experience.
- CPR certification.
