Job Summary
">We are seeking a skilled Registered Nurse (RN) to join our Providence Home and Community Care (HCC) team as a RN Clinical Care Coordinator. This role is responsible for coordinating care and services for our elderly patients with complex medical and psychosocial issues. As a key member of the interdisciplinary team, you will work closely with healthcare providers to ensure seamless coordination of care and improve patient outcomes.

Requirements
">To be successful in this role, you must have:
">A current Oregon Registered Nurse License
National Provider BLS certification from the American Heart Association within 30 days of hire
Relevant certifications applicable to the position, as specified by the Nurse Manager
