About the Role:

The successful candidate will be responsible for screening patients for post-acute needs, transitioning them through the continuum of care, and coordinating their care plans. They will work collaboratively with the healthcare team to ensure seamless patient care and satisfaction.

What You'll Do:

  • Screen patients for post-acute needs: Assess patients' needs and develop personalized care plans to meet those needs.
  • Transition patients through the continuum of care: Coordinate patient transfers between departments and facilities to ensure smooth transitions.
  • Coordinate care plans: Work with the healthcare team to develop and implement comprehensive care plans that address patients' physical, emotional, and social needs.

Requirements:

To succeed in this role, you will need:

  • A nursing degree or diploma
  • An Oregon Registered Nurse License
  • 2 years of acute care hospital or community-based experience
  • 1 year of experience with electronic medical records or documentation systems
  • 1 year of home health, mental health, substance use, hospice, utilization review, discharge planning, or care management experience

Preferred Qualifications:

We also welcome candidates with:

  • A bachelor's degree in nursing
  • Clinical Case Management (CCM) certification

Salary Information:

The estimated salary range for this role is $49.77 - $73.13 per hour, depending on experience and qualifications.
