Key Responsibilities:

  • Screens for post-acute needs and develops effective transition plans
  • Collaborates with the continuum care team to coordinate care and ensure seamless transitions
  • Provides education and community care referrals to support patient self-management
  • Monitors care plan effectiveness and reassesses the need for Care Management services

Requirements:

  • Nursing degree/diploma required upon hire
  • Bachelor's degree in Nursing or higher within 3 years of hire
  • Oregon Registered Nurse License upon hire
  • 2 years of Acute care hospital or community-based experience
  • 1 year of experience with electronic medical records or documentation systems
  • 1 year of Home health, mental health, substance use, hospice, utilization review, discharge planning, or care management experience
