Job Overview

We are seeking a highly skilled and dedicated Patient Care Manager/RN to join our team in Portland. As a key member of our Home Health team, you will be responsible for the supervision and coordination of clinical services, ensuring that patients receive high-quality care in their homes.

About the Role

The successful candidate will have current RN licensure in Oregon, CPR certification, and a reliable vehicle. With a strong background in Home Health experience, you will be responsible for supervising and coordinating clinical services, evaluating patient orders, and plotting start-of-care visits.

Responsibilities

  • Supervise and coordinate clinical services to ensure high-quality patient care
  • Evaluate patient orders and plot start-of-care visits
  • Coordinate determination of patient home health benefits and medical necessity
  • Ensure patient needs are continually assessed and care rendered is individualized to patient needs

Requirements

  • Current RN licensure in Oregon
  • CPR certification
  • Reliable vehicle and access to public transportation

Benefits

LHC Group offers a competitive salary range of $ $85,000 - $110,000 per year, depending on experience, plus comprehensive benefits package including health, dental, and vision insurance, 401(k) retirement plan, and paid time off.
