As part of our new initiative, we are launching the Fulton Community Reentry Center, which will offer 140 transitional housing beds for men exiting incarceration who face homelessness. The center will also provide comprehensive onsite reentry services, including case management, benefits enrollment, employment assistance, health services, and substance use disorder treatment. Our facility operates 24/7, including evenings and weekends, with some roles requiring flexible hours.

We are currently seeking six full-time Case Managers. The Case Manager will oversee case management services for clients and maintain essential documentation for individuals experiencing homelessness. This role reports directly to the Case Manager Supervisor at the Fulton Community Reentry Center.

Compensation: $27.47-$28.02 per hour

Key Responsibilities:
  • Deliver direct client services, including intake, engagement, assessment, service plan formulation, referrals, advocacy, counseling, exit summaries, and follow-up.
  • Manage a diverse caseload effectively.
  • Track and document client progress towards service plan objectives.
  • Record all interactions with clients or on their behalf.
  • Stay informed about available program resources both onsite and offsite.
  • Facilitate group counseling sessions as directed by the Director of Social Services.
  • Coordinate client services with various social service, mental health, medical, employment, educational, and other providers.
  • Conduct regular case conferences with clients and service providers as necessary.
  • Meet with the supervisor bi-monthly to report on progress.
  • Ensure compliance with all relevant regulations and organizational policies.
  • Provide crisis intervention and stabilization as required.
  • Perform additional tasks as assigned.
Qualifications:
  • Associate's degree in social work or a related field.
  • Minimum of one year of case management or relevant experience.
Desired Skills:
  • Engaging personality with strong attention to detail.
  • Excellent oral and written communication, time management, and organizational skills.
  • Willingness to work some evenings and occasional weekends.
  • Proficiency in CARES, MS Word, Excel, and database management.
  • Familiarity with community resources.
  • Problem-solving skills and the ability to remain calm in crisis situations.
  • Proven ability to collaborate effectively with diverse groups.
  • Capacity to manage multiple tasks efficiently under pressure.
